Our partnership is improving health and social care through integration, so local people can lead healthier and happier lives.

In order to achieve this the partners are working together on a number of projects designed to enable high quality integrated care, support the overall sustainability of the local health and social system, and make sure people receive the right care, in the right place, at the right time.

Southwark and Lambeth Integrated Care uses a methodology called the Model of Change. The Model of Change is a clearly defined framework designed to ensure the successful delivery of projects. The Model enables the development, testing and evaluation of new ideas and approaches to care. Through using this approach, it provides consistency in how projects are managed and helps to inform whether projects are of a high enough calibre to be rolled out across the two boroughs.

It is based upon three distinct phases, so each project focuses on the objectives it set out to achieve. The three phases are:

  • Scoping
  • Design and testing
  • Adaption and adoption at scale

Model of Change

Scoping phase – All projects will start by fully exploring specific health and social care issues, problems and challenges being faced. During this phase a range of ideas to address the issues are identified and developed. The ideas and solutions are explored in more detail to determine which should be focussed on and taken through to the design and testing phase.

Design and testing phase – This is where the ideas and solutions are developed and tested, to help refine them into approaches that successfully deliver the desired outcomes required from a new services and ways of working. In this phase it may become apparent that the idea generated within the scoping phase does not deliver improved outcomes when tested and a decision can be made on stopping this project from progressing further.

Adaption and adoption at scale phase – This is the final phase of the Model of Change, where the new services and ways of working are further adapted, building on the lessons from the testing phase. Following this further adaption, the projects are rolled out to be adopted on a wider scale within Southwark and Lambeth.

Feedback loops – These represent the continuous refinement and improvement that occurs throughout the Model of Change process for new services and ways of working.
