Community Multi-Disciplinary Teams
Community Multi-Disciplinary Teams are groups of health, social care, voluntary and community sector professionals that come together to discuss and improve complex care plans of people in Southwark and Lambeth.
The group meet monthly to support Integrated Care Managers, GPs and other professionals manage complex care in the community. It is an opportunity for a variety of professionals to suggest changes and solutions to the care being delivered that may not have been previously thought of.
The meetings are chaired by a local general practice professional and are attended by: geriatricians, district nurses, social workers, therapists and mental health professionals from local health, social care, voluntary and community sector organisations.
The six CMDTs set up across the two boroughs have helped knock down barriers and create an open channel of communication between the different professionals that did not exist before.
