Older People’s Information Directory

The aim of this project was to create an online directory of clinical, social and voluntary sector support services for older people, carers and health and social care professionals in Southwark and Lambeth.

This was required because of the huge range of local services provided and the difficulty in accessing them, particularly the smaller voluntary sector organisations, such as lunch clubs that have limited resources to promote themselves. 

The starting point for the project was to help people live well with Dementia by improving awareness and access to local Dementia services. Following successful testing, the directory was expanded to include all types of services that support older people, ranging from exercise classes to housing advice and social clubs. There are now over 1,800 services promoted on the directory. 

The directory, hosted on the Age UK Lambeth website, helps support older people and their carers to access local services to reduce isolation and loneliness, and so prevent and reduce the impact of crises, which could result in hospital admissions and residential care.

It will also help health and social care professionals to access information about services not previously on their radar, understand how services operate and consider alternatives to traditional services.  This supports the move towards a social prescribing model by reducing the reliance on medical services through supporting the person more holistically and addressing their well-being.
