This project aims to help local people navigate the health and social care system and enhance the quality of delivery of our three main primary care programmes; through Community Multi-Disciplinary Teams, Holistic Assessments and Integrated Care Management.
The project, which is focused in North and South Southwark, has two key elements; they are:
- Developing the Voluntary and Community Sector to be able to play an active role in the delivery of proactive and integrated care in Southwark. This will be done by introducing three Safe and Independent Living (SAIL) navigators, who will be employed and trained by Age UK Lewisham and Southwark, and integrating them into GP practices initially in South Southwark and then in North Southwark.
- Supporting and up skilling reception, administration or counter staff in 10 GP practices and 10 community pharmacies as primary care navigators in North Southwark. These navigators will work with the SAIL navigators to spread the impact of effective care navigation and support the needs of older people.
The project has been co-designed with input from the GP Federations in Southwark, Age UK Southwark and Lewisham, Southwark CCG, local GP practices and the Resilience Interest Group. It will be led by both GP Federations in Southwark (Quay Health Solutions and Improving Health Limited), in partnership with Age UK Lewisham and Southwark.
