Local Care Record coming soon…
This winter, we are introducing a new way of sharing patient electronic medical records securely between hospitals, community services and GP practices.
The Local Care Record will allow staff directly involved in a patient’s care at Guy’s and St Thomas’, King’s College Hospital and South London and Maudsley NHS Foundation Trusts, plus GP practices in Southwark and Lambeth, to view each other’s records safely and securely at the touch of a button.
The three hospital organisations already share information quickly and securely via King’s Health Partners (KHP) Online. KHP Online was recently shortlisted in the Enhancing Care by Sharing Data and Information category in the 2015 Health Service Journal (HSJ) Awards.
The Local Care Record means that this can now be expanded and opened up to allow shared information between GP practices, hospitals and community based care.
It is a secure view-only clinical information portal and is only for the purposes of direct provision of care or treatment to a patient.
It will allow healthcare professionals to view test results, medication, previous treatment and any other clinical information which might help improve the care they can provide for people.
Treatment will be safer and quicker as healthcare professionals will be able to review all relevant information before making clinical decisions.
Patients and carers will no longer have to repeat information to multiple healthcare professionals or wait a long time for test results.
The Local Care Record has the potential to transform the way we provide care for people in Southwark and Lambeth. By working together with our local partners we will treat our patients more quickly, safely and effectively. We will have a more complete understanding of all of their health needs, and will be able to provide them with a more joined-up journey through our local NHS services.
There are now over 60 GP practices signed up and this covers nearly 70 per cent of the population (as of 9 December 2015).
The new system will start to connect a small number of GP practices with the hospital organisations from winter 2015, and other GP practices will continue to join during 2016.
